Inside every knee, the joint lining — a thin, silky membrane that lubricates and nourishes the joint — sometimes folds back on itself during fetal development. These folds, called plicae, are present in somewhere between 25% and 80% of knees. For the vast majority of people they remain soft, pliable, and entirely painless throughout life. In a small minority — estimates range from 3.8% to 5.5% of the population — one of these folds becomes a problem.

The fold that matters most clinically is the medial plica, which runs along the inner wall of the knee and sits between the kneecap and the inner end of the thigh bone. Under normal conditions it glides freely. After a direct blow to the knee, a twisting injury, or months of repetitive loading — running, cycling, repeated squatting — it can thicken and stiffen, losing its pliability and becoming more like a tight cord. With each bend of the knee, this cord catches and pinches between the kneecap and the thigh bone rather than sliding smoothly past.

The result is a recognisable pattern: a dull ache on the inner side of the knee, an audible or palpable click, stiffness after sitting or resting, and intermittent swelling. Symptoms tend to creep up gradually rather than beginning with a single injury.

In cases where the pinching goes unrecognised for months or years, the repeated friction can wear away cartilage on the inner surface of the kneecap — a structural consequence that makes early identification worthwhile.

Why plica syndrome is so often missed

Getting to a plica diagnosis frequently takes time — not because clinicians are missing something obvious, but because the condition is a structural mimic. The ache on the inner side of the knee, the clicking, the stiffness after sitting: these are also the cardinal features of patellofemoral pain syndrome, a small meniscal tear, and chondromalacia patellae. Clinicians reach for those diagnoses first because they are far more common, and standard X-rays — almost always the first investigation — return entirely normal results, pointing nowhere.

When an MRI is eventually arranged, the picture can mislead in a different direction. Plicae are present in the majority of knees, so a fold appearing on a scan is not by itself a diagnosis — symptomatic disease requires clinical correlation alongside the imaging finding. Many patients cycle through one or more explanations before the plica is even considered, which is why some describe years of anterior knee pain with no satisfying answer.

One clinical detail cuts through the ambiguity more reliably: the exact location of the pain. Plica syndrome produces tenderness above the joint line — the palpable crease across the front of the knee — whereas meniscal pathology is characteristically felt at the joint line itself. How plica syndrome is diagnosed

Assessment typically begins with a hands-on examination before any scan is requested. The mediopatellar plica (MPP) test involves the clinician palpating the medial plica while slowly flexing and extending the knee — reproducing the patient's familiar catch or click is a positive finding. In published comparisons, the MPP test has been shown to outperform MRI as a tool for identifying a symptomatic plica, making the clinical examination a frontline step rather than a formality.

MRI follows as the primary imaging investigation — not because it confirms plica syndrome reliably, but because it is essential for ruling out the conditions it can mimic: meniscal tears, ligament injury, chondromalacia patellae. Its limitations in this specific context are worth understanding: a 2014 meta-analysis by Stubbings and colleagues reported a sensitivity of 0.77 and a specificity of 0.58 for positively identifying a symptomatic plica — figures that fall short of most diagnostic thresholds. A plica visible on MRI still requires symptom correlation before it carries clinical weight.

Dynamic ultrasound is an increasingly used adjunct that addresses some of MRI's shortcomings. Because it images the plica in real time as the knee moves, it can capture the impingement directly; in a 2009 RSNA study by Paczesny and colleagues, dynamic ultrasound achieved 88% overall accuracy, 90% sensitivity, and 83% specificity for medial plica syndrome — substantially higher than MRI in this role.

Arthroscopy sits at the end of the pathway, reserved for when earlier steps have not resolved the clinical question. It is the diagnostic gold standard: a surgeon can directly visualise the plica, assess its thickness and fibrosis, and — where appearances and symptoms align — excise it in the same procedure. It is not a first-line investigation, but the fact that it can both confirm and treat in a single step means it becomes a logical next move when conservative management has failed.

Conservative treatment and what to expect

Most patients with plica syndrome do not need surgery. A 2025 systematic review and meta-analysis by Franco and colleagues — pooling six clinical trials and 527 patients — found that conservative management produced functional improvements statistically indistinguishable from arthroscopic excision at 12–24 months follow-up. The pooled Lysholm score improvement with non-surgical care was 24.5 points (95% CI 18.7–30.2), a clinically meaningful gain that the evidence now supports as the baseline to aim for before surgical referral is contemplated.

The core programme involves four elements: activity modification to reduce provocative loading, anti-inflammatory medication (NSAIDs) during acute flares, range-of-motion exercises to maintain joint mobility, and targeted physiotherapy. The physiotherapy component matters most mechanically — a programme centred on quadriceps strengthening and hamstring flexibility, as outlined by Griffith (2007), reduces the compressive and shear force that the plica exerts on the medial femoral condyle and patellar facet during movement. Addressing that mechanical impingement, rather than simply resting the knee, is what drives sustained improvement.

The standard threshold before surgical referral is considered is three to six months of consistent, structured conservative management. Improvement is gradual; some patients notice meaningful change within six to eight weeks, others take considerably longer. Commitment to the programme influences the outcome more than any single component within it.

One acknowledged limitation in the evidence is that no universally validated rehabilitation protocol exists. Programmes vary between centres, and the Franco 2025 review noted this as a gap. For that reason, guidance from a physiotherapist with specific knee expertise — rather than a generic injury programme — is worthwhile. A proportion of patients will not respond adequately to conservative care, and for them the question of arthroscopic excision becomes appropriate.

When arthroscopic excision is the right step

For the minority of patients whose symptoms persist after a structured conservative programme, arthroscopic plica excision offers a reliable next step. The procedure involves keyhole removal of the thickened or fibrotic plica fold under direct visualisation — which also resolves the diagnostic ambiguity that imaging sometimes leaves open: the surgeon can see the plica, assess its extent, and excise it in the same sitting.

Published outcomes support this escalation. A 2018 meta-analysis by Gerrard and colleagues found good-to-excellent results in 84.2% of knees following arthroscopic resection (95% CI: 72.8–91.4%), with approximately 60% of patients achieving complete resolution of mechanical symptoms. Recovery to full activity takes around four to six weeks for most patients — a comparatively low burden relative to other knee interventions — though individual timelines vary and cannot be stated as a guarantee.

Surgery in this context is not a failure of conservative care; it is the appropriate escalation when a structured programme has been thorough and consistent and has not produced adequate relief.

One honest caveat: the evidence base for arthroscopic plica excision, while broadly favourable, currently provides limited data on long-term recurrence rates and durability beyond two years. That gap does not diminish the short-to-medium-term evidence, but it is a reasonable question to raise with a surgeon before proceeding — and a realistic expectation to carry into any post-operative follow-up.
