What jumper's knee actually feels like
The most telling moment usually comes on landing — a sharp, localised ache at the very bottom of the kneecap that either stops you mid-session or lingers for hours afterwards. That spot, the inferior pole of the patella, is where the patellar tendon anchors to bone, and in jumper's knee it becomes the focal point of every load the leg absorbs.

Escalation does not signal that physiotherapy has failed. Red flags and when surgery becomes relevant
Three clinical signs demand same-day attention, regardless of where someone is in their rehabilitation programme.
A palpable gap or step in the tendon, a sudden loss of the ability to straighten the knee against gravity, or a patella that sits abnormally high (patella alta) each point toward complete patellar tendon rupture rather than ongoing tendinopathy. Unlike tendinopathy, rupture is a surgical priority: the interval between injury and repair is the single most important factor governing outcome. Repair is typically achieved through transosseous tunnels or suture anchors, restoring continuity of the extensor mechanism; return to sport after uncomplicated repair is generally anticipated around six months post-operatively, progressing through criteria-based functional milestones rather than a fixed date.
For the smaller group of patients with degenerative — non-ruptured — tendinopathy that has genuinely exhausted structured exercise rehabilitation, shockwave therapy, and PRP, surgery remains available as a final resort. Evidence supports this step only after the full conservative and injection-based pathway has been completed and documented.
There is also a practical reason not to leave a stalled conservative pathway unaddressed: long-standing, uncontrolled tendinopathy does increase the susceptibility of a chronically altered tendon to low-energy rupture.
